IMPORTANT NOTICE
ALL Students that are promoting to the 7th grade are required by the State of Texas to have the following immunizations:
-Tdap Vaccine (Tetanus/Diphtheria/Pertussis)
1 dose of Tdap is required if at least 5 years has passed since the last dose of the tetanus containing vaccine.
--Meningococcal Vaccine
1 dose of the meningococcal vaccine is required on or after the student’s 11th birthday.
NOTE if a student received the vaccine at 10 years of age, this would satisfy the requirement.
Passport Health/Staying Health Medical Services will be conducting vaccination clinics to get your students up to date.

A student shall show acceptable evidence of vaccination prior to entry, attendance, or transfer to a child-care facility or public or private elementary or secondary school in Texas.
